Delta Dental sends new employee cards

Delta Dental, which provides dental insurance plans for ITD and other state employees distributed new membership cards this week that replace Social Security numbers with personalized numbers.

“To protect your identity, the State of Idaho has requested [that] each employee receive a new identification card. Instead of using your Social Security number, each employee has been assigned an individual subscriber number,” according to an announcement from the company.

Updated benefit identification cards have been reissued. Information on the new cards includes the name of the employing agency, group number, employee’s full name, and a new subscriber number. Employees are asked to shred their old cards and begin using the new ones immediately.

Letters distributed this week to all participating employees also included a summary of their coverage plan.

The March 27 letter also contains an updated Notice of Privacy Practices that governs Delta Dental policies regarding the release of personal health information to outside entities.

State employees who would like information about dental benefits and better oral health sent to them by e-mail can make that request at eligibility@deltadentalid.com . Indicate in the request that “I would like to obtain notification of my Explanation of Benefits electronically.” Include the e-mail address where future correspondence should be sent.

The company does not sell, share, rent or lease personal information to third parties.
