Employees encouraged to make sure
personal data is current

As the end of 2006 – and the present federal tax year – approaches, ITD employees are encouraged to ensure their address is current. If any changes have occurred the past year or employees are not sure of the information on file, they should contact Human Resource Services (Headquarters employees) of district business offices.

The State Controller’s Office (SCO) will mail W-2 forms to the address on file as of Jan. 5, 2007. In preparation for that mailing, changes must be submitted to Human Resources (HRS) or district business offices by Dec. 8. Employees who do not receive their W-2 forms by Jan. 5 may request a duplicate from the SCO at a cost of $5. Requests must be made in writing and signed.

The state again will allow W-2 information to be downloaded into participating tax preparation computer packages for 2006. The calendar year 2006 W-2 download file will be available on the Internet in late January.

Employees eligible for earned income credit must complete and sign both a 2007 W-5 form and a new EIS-EIC form for the coming year. The SCO will not accept 2006 W-5 forms that have been altered to read 2007. Please contact HRS or your district business office if 2007 forms are needed. The forms are not currently available, but should appear soon on the IRS Web site.

Please take time to make sure your PERSI, 401k, and state-sponsored life insurance beneficiary information is up to date. Employees can check their listed beneficiaries by contacting HRS or the appropriate business office.
