Changes announced for employee evaluation forms

HRS recently announced the revision of the performance evaluation form and has just completed delivering a series of supervisory trainings and employee orientations throughout the districts and Headquarters. During those sessions, feedback was requested and compiled, and HRS has carefully considered all comments prior to releasing the form.

Some things will be addressed immediately, prior to release. An example is the addition of technical language to the rating criteria; this will especially reflect some Division of Highways concerns regarding rating equipment usage, etc. HRS will assess the form over the next year, and potential revisions will be considered after feedback and observation data has been evaluated.

Transitioning to the new form
If you have an employee whose performance evaluation is due within the next few months, HRS suggests that supervisors check with their employees to determine if they would prefer to be closed out on the old, rather than the new form. Following the evaluation, regardless of the form used, a new performance plan should be set up using the new format. For those employees due later, HRS suggests using the new form. You will not be able to close out employees before their annual review date.

Employee concerns
Employees with concerns about their ratings can write and attach comments to the form at the time they sign it. Additionally, they can write, ‘see attached employee comments’ with their signature.

Additionally, formal problem-solving is an option to all employees for various work-related concerns, including those regarding performance evaluations by completing an ITD-1205 Problem Solving Request Form. The employee should keep a copy of the completed ITD-1205 and send a copy to the HRS manager.

The form will be available on Form Finder, #1100, Nov. 9. It will be accompanied by a quick reference guide for ease of use. If, however, you have questions as you use the form for the first time, HRS staff will be available to provide assistance. Contact numbers:
